Full time MDS Coordinator
The Port Chester Nursing & Rehabilitation Centre is a resident centered facility located in southern Westchester County, NY. We are dedicated to a culture based on honesty, integrity, diversity and the highest ethical practice.
We are currently seeking an MDS Coordinator to join our caring and dedicated team.
Responsibilities include:
• Responsible for the accurate and timely completion of all Medicare/Medicaid MDS 3.0 documents in order to assure appropriate reimbursement for services provided within the facility.
• Conduct and coordinate the development and completion of the resident assessment (MDS 3.0, CAA’s, Care plans) following state and federal guidelines governing LTC.
• Chair the facility’s Comprehensive Care Plan meetings.
Job requirements:
• Minimum 2 years experience as an MDS coordinator in LTC/SHF setting
• Current license to practice as an RN in New York State.
• Extensive knowledge of RAI process is required.
• Medicare and Medicaid experience required.
• Good computer skills including data entry, RUG (Resource Utilization Groups) calculation.
• Strong communication, organizational and time management skills.
Very competitive salary and benefits commensurate with experience.

7am-3pm shift, 3pm-11pm shift and 11pm-7am shift
